What to include in a wound-care referral

A useful referral identifies the patient, location, wound concern, responsible clinician, requested timing and direct contact for follow-up.

Send enough information to review the request

Name the clinician or organization responsible for current orders

Use a secure channel for records and photographs

Basic referral information

The public referral form should contain only what Gateway needs to respond.

  • Organization, referrer name, title and direct contact
  • Patient initials and city or ZIP
  • Home, facility or discharge location
  • General wound type, recent change and requested timing

Clinical records after initial contact

Gateway will provide a secure method when records are needed.

  • Current wound-care orders
  • Recent measurements and treatment history
  • Relevant discharge, surgical, vascular or podiatry notes
  • Payer and authorization information when applicable
Avoid referral delaysList one direct contact who can answer clinical questions and receive Gateway’s response.
