Public website forms should contain only basic contact and referral information. Detailed records and photographs should use the secure channel provided by the clinical team.

Before sending information

Confirm the purpose, recipient and secure method.

  • Use patient initials rather than full identifiers in the public form
  • Wait for Gateway to provide the appropriate channel
  • Confirm the direct clinical or referral contact
  • Send only records relevant to the wound-care request

For wound photographs

Images should be clinically useful and linked to the correct patient without exposing unnecessary information.

  • Follow the organization’s privacy and consent policy
  • Use consistent lighting and a measurement scale when requested
  • Label the date and wound location
  • Do not rely on a photograph when symptoms require immediate evaluation
